Please Note: These facilities are not part of Flora's standard Preferred Provider Organization.  Approval is required prior to obtaining services at any of these providers.


If you are a transplant candidate or have an interest in accessing one of these network providers, please contact your health insurer for more information.  Since not all Health Benefit Programs have access to this service it is IMPORTANT to have your benefits manager, or care manager, and provide the program number for assistance.
